Feel Great Weight Contest
OFFICIAL RULES
To Enter:
To enter, submit your Feel Great Weight story at www.health.com/feelgreatweight/ by using the online entry form provided. Entries received online must be received by 11:59 p.m. EST August 31, 2008. You may enter as many stories as you like. A separate entry form must accompany each story. Entries must be original and not previously published. Story must be legibly printed or typed, and must include your name, address, e-mail address, and phone number. Incomplete or illegible entries will be disqualified. All stories become property of Health Magazine and will not be returned. By entering this contest, you grant Health Magazine and Sponsors the right to edit and publish the story, with credit, in Southern Progress Corporation publications, products, and online services, and those of its corporate affiliates. Confirmation of receipt of entry is not available.
Judging:
All entries will be judged by representatives of Health magazine based on creativity (25%), originality (25%), and compelling story (50%). In the event of a tie, the person among the tied entries with the highest score in creativity will be declared the winner. Your submission must be original and not previously published. Decisions of judges are final and binding. Winner will be notified by email, telephone and/or mail. 26 stories will be selected by Health Magazine representatives as finalists. One Grand Prize Winner will be selected from among the 26 finalists. Finalists will be required to sign statement of eligibility and release of liability prior to receipt of prize. By acceptance of the prize, each finalist consents to use of his or her name and/or photograph for publicity purposes without further compensation. If for any reason the Contest is not able to be conducted as planned because of circumstances or causes which in Health Magazine's sole opinion, affect the security, fairness or proper conduct of this event, Health Magazine reserves the right to modify the location, structure or nature of the Contest.
Categories and Prizes:
25 finalists will each be awarded a Feel Great Weight Kit which consists of one $50 iTunes gift card, coupons equaling a 6-week free supply of Skinny Cow ice cream, and one Sportsline Thinq pedometer from Special K (ERV:$100). 1 Grand Prize Winner will be awarded a special 6-month membership to Equinox Fitness Clubs (ERV: $1,152). Finalists will be notified no later than September 30, 2008. Any finalist who cannot be easily located at the address and phone number provided will forfeit the chance to compete. Prize winners are responsible for taxes on the value of all prizes awarded.
Who Can Enter?
Entrants must be legal residents of the United States who have reached the age of majority in their state of residence at the time of entry. Employees of Southern Progress Corporation, sponsoring companies, their affiliates, subsidiaries and agencies, or any member of the immediate family (parent, children, siblings, spouse, regardless of where they live) or same household of such employee (whether related or not) are ineligible. By entering you agree to be bound by the terms of these Official Rules and by the decisions of Health Magazine, which are final and binding on all matters pertaining to this Contest.
Miscellaneous: No substitution or transfer of prizes, except at the discretion of Health Magazine. Health Magazine is not responsible for misdirected or delayed mail, lost or late entries or for electronic transmission errors resulting in omission, interruption, deletion, defect or delay in operations or transmission, theft, destruction, unauthorized access to or alteration of entry materials, or for malfunctions or limitations of any kind. If for any reason the Internet entry portion of the Contest is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Health Magazine which affect the administration, security, fairness, integrity, or proper conduct of this Contest, Health Magazine reserves the right at its sole discretion to cancel, terminate, modify or suspend the Contest. Health Magazine reserves the right to select winners from eligible entries received as of the termination date. Void where prohibited by law. All federal, state, and local regulations apply. Entrants agree to be bound by the terms of these Official Rules and by the decisions of Health Magazine, which are final and binding on all matters pertaining to this Contest. Names of Category and Grand Prize winners will be posted on our Web site following the final competition.
